With Synap's results override features you can modify and attempt and track those changes through an exam attempts history, let team members leave comments around changes.

In any exam with a completed or finalised attempt, select a user in the exam management table and then the override button.
From here a modal will open which lets you modify the attempts: points, overall score and grade. Leave comments, view previous changes and revert grades, points or score back to their original values.
